Our Access & Referral (A&R) team provides up-to-date access, information and support regarding SEMPHN-funded mental health and alcohol and other drug services.
The Access & Referral team also has extensive knowledge of other local community and health supports.

Instructions for downloading and uploading referral forms
- In order for the PDFs to remain writable please access them via Google Chrome.
- Right click on link and select ‘Save Link As’.
- Select a location on hard drive (which you can find later, such as the desktop) and save.
- Fill in the referral form digitally and save.

Best Practice
- After downloading the .zip folder, open the folder and 'Extract' all the files to a folder on your computer.
- Open a New Letter and from Templates menu select Import Template
- Locate downloaded file and open
- From File menu select Save as. Type an appropriate Template name and click on Save.
ZedMed
- After downloading the .zip folder, open the folder and 'Extract' all the files to a folder on your computer.
- Click on Tools > Clinical WP Setup > WP Templates
- File > Import Template
- Find the file where you saved it and highlight it and then click Open
- Enter the template name into the Description. Click on OK.
Medical Director
- After downloading the .zip folder, open the folder and 'Extract' all the files to a folder on your computer.
- Open Medical Director and then LetterWriter
- File menu > Select Modify Template > Select Black template
- File > Select Import - locate downloaded file and click on Open
- File > Select Save as template - type in an appropriate name and click on Save
