Health care Homes First Stage Commences

The first stage of the Australian Government’s Health Care Homes program began on 1 October with eight general practices in the South Eastern Melbourne catchment rolling out the model and beginning to enroll patients:
- Lynbrook Village Medical Centre
- Pearcedale Medical Centre
- First Health Medical Centre – Hampton Park
- Thompson Road Clinic
- Lakeside Square Medical Centre
- Hallam Family Practice Pty Ltd
- Carrum Downs Medical Centre
- Dandenong West Medical Centre
The Health Care Homes model ensures people with chronic conditions receive ‘wrap around’ care, with a local health care team acting as the home base for a patient’s overall care and coordination.
An additional 12 general practices in the South Eastern Melbourne region will begin the stage one rollout on 1 December 2017.
SEMPHN has a dedicated team of Practice Facilitators working with the practices which are participating in the Health Care Homes program.
