Better Health Care Connections (BHCC) – Aged Care Multidisciplinary Care Coordination and Advisory Services

SEMPHN is one of nine sites across Australia selected to run the Commonwealth BHCC program. A significant focus of the program is the demonstration of Telehealth to increase access to timely and appropriate care for residents of aged care facilities.


GP-RACF video consultation pilot

The GP telehealth program is being piloted in around 30 Residential Aged Care Facilities (RACFs) across Australia, with four on the Mornington Peninsula; Andrew Kerr Care; Somercare; Craigcare-Mornington and Ti Tree Lodge.

All GPs with patients who reside at these facilities are eligible to participate in paid video consultations to demonstrate the effectiveness of this tool for the aged care sector. Peninsula Health Clinical Response Service (CRS) is also engaged. This program ends June 2017.


Specialist Telehealth

Medicare Benefit Schedule (MBS) items exist for Specialists to undertake video consultations with patients in RACFs anywhere in Australia. GPs can also claim for patient–end support. A list of telehealth capable Medical Specialists providing care for residents in the Peninsula Health catchment is available for RACFs and GPs.


Nursing and Allied Health Telehealth Pilot

SEMPHN and Peninsula Health ROSS- Residential-in-Reach (ROSS–RIR) have a short term pilot of telehealth support for wound care, falls management and advance care planning. This pilot ended Dec 2016.
