NOTE: This event is for Practice Nurses working in the SEMPHN catchment. 

Event Overview

This education program and has been developed specifically for Practice Nurses and is being delivered as part of SEMPHN's Practice Networking Meetings.

Practice Network Meetings provide an opportunity for Practice Nurses across the region to come together to learn and network.

Practice nurses, whether your experience with health assessments and care planning is novice, intermediate or experienced, this Practice Nurses Network is for you!

The October Practice Nurse Network will welcome speaker Brooke Gilbert who will present on Health Assessments and Care Planning.  Brooke is a Registered Nurse and has worked in general practice for over nine years. She has developed and implemented chronic disease management programs in her clinics, resulting in better outcomes for her patients and increases in general practice revenue.

This is an opportunity for you to network with other practices, and learn about:

Health Assessments

  • Identifying patients at risk and using Polar or PENCAT to work out where to focus your energies
  • MBS requirements and components for chronic disease management, diabetes
  • 45-49 Health Assessment
  • Over 75 Health Assessment
  • Comprehensive Medical Assessment (CMA) for patients in Aged Care facilities

Care Planning

  • Who to target, focus on, goal setting
  • Team Care Arrangements (TCA)
  • General Practice Management plans (GPMP)
  • Cycles of Care
  • MBS Billing

Please note: Practice Managers have a concurrent education/networking activity at the same time and at the same venue. 


Participants will receive a certificate of attendance for 2 hours of education.


Free but registration is essential. A light meal will be provided from 8:00am. Presentations will begin promptly at 8:30am.
