Care Co-ordination Funding EOI
Come to an information night on 19 February to hear about one of our biggest funding opportunities to date. Only practices who send a representative to the information session will be eligible to apply for the EOI.
SEMPHN is offering up to $250,000 per General Practice to deliver care co-ordination for patients with chronic conditions and/or those who identify as a refugee. Identifying and planning for each patient's after hours health care needs is a key component of the funding.
