Grants open to support After Hours innovation

SEMPHN invites organisations to apply for one-off funding grants from $100,000 (ex.GST) to support innovative After Hours (AH) primary care initiatives in our catchment.
The new activities will aim to reduce General Practitioner-style presentations to Emergency Departments After Hours.
The initiative will need to address one or more of the following:
- Improve access to primary health care services within the After Hours period
- Improve the effectiveness and efficiency of primary health care services within the After Hours period
- Complement After Hours general practice services through substitution and/or diversion initiatives, and/or,
- Reduce the demand of primary GP style presentations to Emergency departments.
The grants are offered for activities across our whole catchment, but priority may be given to suitable proposals in the LGAs of Dandenong, Cardinia, Casey, Mornington Peninsula and Frankston.
