Is your General Practice in South Eastern Melbourne thinking of taking on care co-ordination? Or, does your practice want to take care-ordination to the next level?

Come to an expression of interest information night on 19 February to hear about one of our biggest funding opportunities to date.

SEMPHN is seeking expressions of interest for funding up to $250,000 per General Practice to deliver care co-ordination for patients with chronic conditions and/or those who identify as a refugee. Identifying and planning for each patient’s after hours health care needs is a key component of the funding.

  • Only practices who send a representative to the information session will be eligible to apply for the EOI.

Please note: General Practices who are contracted to the Health Care Homes Trial – either now or in the last six months, and General Practices who are, or have been, participating in the SEMPHN WellNet Integrated Care Program, are NOT eligible to apply for this EOI.
