These Dementia Pathways are an online list of dementia-related services, supports and resources in south eastern Melbourne. The information on this page is for people living with dementia in the following local government areas: Bayside, Cardinia, Casey, Frankston, Glen Eira, Greater Dandenong, Kingston, Monash, Mornington Peninsula, Port Phillip, and Stonnington.

First steps after receiving your dementia diagnosis

Below are some useful steps to take after finding out about your dementia diagnosis.

Step 1: Speak to your GP

  • Speak to your GP about treatment and options for managing dementia.
  • Ask for dementia resources from your doctor.
  • Ask about specialist appointments, allied health, and advanced care planning.

Step 3: Call any relevant helplines

There is a list of helplines available to call that offer a wide range of support - access them by visiting our Dementia Helplines page.

Step 4: Access Government, social and community support

You could access support from My Aged Care, Services Australia, community organisations and your local council.

Medical care to treat dementia

This section provides a list of dementia specific medical services to treat and care for people living with dementia. 

Cognitive, Dementia and Memory Services

The Cognitive Decline & Memory Service (CDAMS) can provide detailed assessment, a diagnosis or explanation of memory and thinking problems that are causing you or your family concern. CDAMS clinics do not offer ongoing treatment post-diagnosis, information, advice and referral on to other services can be provided for any ongoing treatment or support needs.

Aged Care Assessment Services (ACAS)

The Aged Care Assessment Services determine your eligibility for subsidised aged care. A two-part assessment process is undertaken to gain an understanding of what services are best suited to your needs. An eligibility check is conducted online or over the phone, followed by an in-person assessment. 

Click this link to check your eligibility - My Aged Care - Am I Eligible, or call My Aged Care on 1800 200 422. Once your eligibility is confirmed, you can obtain a referral to your local hospital's assessment service.

Alfred Health Aged Care Assessment Service

Alfred Health provides aged care assessment services for people who live in the local government areas of Glen Eira, Port Phillip or Stonnington. 

Monash Health Kingston Aged Care Assessment Service

Kingston ACAS covers the local government areas of Bayside, Dandenong and Kingston

Peninsula Regional Assessment Service

Peninsula RAS covers the Frankston local government areas: Frankston North, Frankston South, Langwarrin, Carrum Downs, Sandhurst, Seaford and Skye

Mount Eliza Aged Care Assessment Service (MEACAS) - Peninsula Health

Allied health services

Using allied health services after a dementia diagnosis has been proven to help manage health and prevent further cognitive and physical decline.

The MyAgedCare website has a service finder you can use to find an allied health service that is right for you. Please enter your suburb or postcode, select "help at home" as the care type. Then click on "Podiatry, physio and other therapies" as the service you are looking for.
