DCCC is available to adults with type 2 diabetes and suboptimal glucose levels (HbA1c >8%), requiring a team approach of different specialists to support their self-management. The clinic is based at the Caulfield Hospital.

The supportive DCCC team will:

- Help to improve their understanding of type 2 diabetes
- Provide clear guidance and instructions to patients to help self-manage their diabetes
- Help patients to learn how to monitor their diabetes, and reduce the risks of developing diabetes complications

The team includes:

- Endocrinologist
- GP Specialist
- Pharmacist
- Diabetes Nurse Educator
- Dietitian
- Exercise Physiologist

After a series of appointments over 6 to 12 months, and once the person with diabetes has achieved their goals, we will transfer care back to their usual GP through a virtual case conference.
