Our Access & Referral (A&R) team provides up-to-date access, information and support regarding SEMPHN-funded mental health and alcohol and other drug services.
The Access & Referral team also has extensive knowledge of other local community and health supports.

How to refer
The referral forms are for the following SEMPHN-funded services: Accessible Psychological Interventions (API), Mental Health Integrated Complex Care (MHICC), BounceBack, Connect, and Commonwealth Psychosocial Services.
