With the right care team, general practices can better support patients with chronic and complex needs.

Funded under the Strengthening Medicare Initiative, the General Practice Multidisciplinary Teams (GP-MDT) program supports 12 solo or small general practices across Frankston and Mornington Peninsula to deliver enhanced care for disadvantaged communities. Practices will employ allied health professionals tailored to meet the needs of their local patient population.

    The GP-MDT program is delivered by the following practices:

    • Headspace Frankston Youth Health Clinic
    • Karingal Family Medical Centre
    • Karingal Hub Doctors
    • Langwarrin Health Clinic
    • Somerville Medical Centre
    • South Coast Medical Dromana
    • St Mark’s Medical Centre Seaford
    • Tyabb Health
  • Who is eligible?

    This program supports patients in the Frankston and Mornington Peninsula LGAs who:

    • Have multiple chronic conditions
    • Are experiencing socio-economic disadvantage
    • Require support with complex care or chronic disease management

    Eligibility is determined by the participating general practices.

  • What support is provided?

    Each practice will recruit allied health professionals to support the needs of their patients with multiple chronic health conditions.


    Allied health professionals can include nurse practitioners, nurses, midwives, and Aboriginal health workers or practitioners.

  • Why is this program needed?

    SEMPHN’s 2023 Health Needs Assessment identified that Frankston and Mornington Peninsula have higher than average rates of people:

    • Living with multiple chronic conditions
    • Experiencing socio-economic disadvantage
    • Requiring income support

    This program provides needed, localised support to improve access to multidisciplinary team care and improve patient outcomes.
