Weave is an integrated mental health and alcohol and other drugs program designed to support people with two co-occurring conditions: a substance misuse disorder and a moderate-to-severe mental health condition.
Typically, mental health conditions and substance use disorders are treated separately, making it difficult for people seeking and navigating treatment. Funded by SEMPHN and delivered by EACH, the Weave Program takes an integrated approach, allowing participants to have both conditions treated by the same practitioners.

The Weave Program is a pilot program available for people residing in Casey and Cardinia LGAs at no-cost and is delivered through centre-based appointments, assertive outreach and telehealth. Support is tailored to individual needs, however it can include:

  • Care coordination
  • Client-driven goal setting
  • Therapeutic/psychosocial interventions 
  • Connection with community-based supports and assistance with service navigation.
  • One on one and group peer support 
  • Family counselling and support 
  • Addiction medicine and other specialist consultations.

The treatment team is multidisciplinary including dual diagnosis clinicians, addiction medicine specialist and lived experience peer workers.

Weave operates within business hours, with after-hours appointments provided as requested (in-person or via telehealth).

  • Who is the Weave Program for?

    Weave is open to anyone who meets the below:

    • Aged 18 years or older
    • Live, work or study in Casey or Cardinia LGAs
    • Have been diagnosed with a co-occurring substance use disorder and moderate-to-severe mental health condition.

    To support priority populations, the program has a strong focus on providing access to treatment for people who:

    • Are experiencing homelessness or housing insecurity
    • Are unemployed
    • Have a culturally and linguistically diverse background
    • Identify as LGBTIQA+
    • Identify as Aboriginal or Torres Strait Islander
    • Have a chronic health condition
    • Are experiencing social isolation.
  • Who can refer?

    Referrals can be made by professionals including GPs and other primary health providers, social services and other organisations.


    Self-referrals and carer referrals are also welcomed.
