Provider Support
Provider Support Officers (PSOs) from the Provider Support team, as well as the Digital Health and Area 4 Pharmacotherapy Network teams, engage with and support practices and services across the SEMPHN catchment.
PSOs regularly engage with and visit general practices to support them in their sustainable business practices and whole of practice patient centred care approaches. Each general practice has an allocated PSO.
PSOs help practices and services understand and take advantage of SEMPHN activities and services and they gather information and data to inform development of SEMPHN activities and services.
The Provider Support team is also involved in a wide range of SEMPHN activities, such as our Practice Networks, Practice Coaching online training, Doctors in Secondary Schools, online Pathways, and Health Care Homes.
