Job Description

We are seeking an experienced and compassionate Nurse Practitioner to provide advanced clinical care primarily to older adults within our general practice and residential aged care facilities.<br>Key responsibilities include:<br>- Conducting comprehensive health assessments for elderly patients.<br>- Diagnosing, treating, and managing acute and chronic medical conditions.<br>- Prescribing medications and ordering/interpreting diagnostic investigations within scope of practice.<br>- Providing regular onsite clinical services to residents of aged care facilities.<br>- Supporting residents and families to navigate aged care and broader healthcare services, including referrals, care coordination, and access to community supports.<br>- Developing and reviewing care plans, including chronic disease management, preventive health strategies, palliative care, dementia care, and complex geriatric care needs.<br>- Collaborating closely with GPs, nursing staff, allied health professionals, aged care staff, residents, and families to deliver coordinated, person-centred care.<br>- Maintaining accurate clinical documentation and ensuring compliance with professional and regulatory standards.<br>The successful candidate will have a strong interest in geriatric and aged care, excellent clinical and communication skills, and a commitment to improving health outcomes and quality of life for older Australians.<br>

Essential selection criteria

- Current registration as a Nurse Practitioner with AHPRA (or eligibility for endorsement).<br>- Strong clinical experience in aged care, geriatric health, or primary care.<br>- Demonstrated ability to assess, diagnose, and manage acute and chronic conditions in older adults.<br>- Prescribing authority and experience in safe medication management.<br>- Ability to develop, implement, and review comprehensive care plans, including chronic disease, dementia, palliative care, and end-of-life care.<br>- Strong communication and interpersonal skills, with the ability to engage effectively with elderly patients, families, and multidisciplinary teams.<br>- Demonstrated ability to support care coordination and assist patients and families in navigating aged care and health services.<br>- Commitment to person-centred, evidence-based practice and continuous quality improvement.<br>- Current CPR certification and relevant immunisation compliance as required.<br>

Desirable selection criteria

- Ability to speak a second language will be highly regarded and considered an advantage.<br>- Experience working in residential aged care facilities and/or community-based elderly care.<br><br>
