CCM Activation Series - Activity 2

Activity 2 of the Chronic Condition Management (CCM) Activation Series will focus on planning with your practice team to ensure they aware and prepared for the upcoming changes.

This activity aims to raise awareness among your practice team of MyMedicare, Chronic Conditions Management changes, and support your team to explore their roles in both MyMedicare and Chronic Conditions Management. By exploring and defining these roles, your practice team can work collaboratively to prepare for change and develop processes, systems and skills needed to succeed. This process will help ease your team through change and provide a shared document that can help your team to identify and discuss anything that isn’t quite working as planned, explore changes and update the document to keep everyone on the same page. This approach will allow your team to adapt and improve and empower each team member in their own role.

Activity outcomes

  1. Your practice team understands MyMedicare (Voluntary Patient Registration)
  2. Your practice team understands the proposed Chronic Conditions Management changes
  3. Your practice team roles in MyMedicare and Chronic Conditions Management are well defined, and each team member has a clear role and responsibilities.

Activities

1)     Engage your practice team to inform them about MyMedicare and the changes to Chronic Conditions Management.

  • Team meeting or quick lunch catch up to communicate the changes
  • Post an update in the practice staff room
  • Send an email to the practice team with the critical information

The MyMedicare GP Toolkit provides a good summary of the current benefits of MyMedicare for General Practices. The toolkit also includes a range of helpful resources your practice can use to communicate with patients.

2)     Engage with your practice team to explore and document team roles and responsibilities related to MyMedicare and Chronic Conditions Management

  • Explore roles and responsibilities with the practice team in a meeting or quick lunchtime discussions
  • Document agreed roles and responsibilities and communicate this with your team
  • Discuss and document how each team member will incorporate their responsibilities into their workday and work week
  • Schedule a time to review your documented roles and responsibilities

South Eastern Melbourne Primary Health Network (SEMPHN) will soon seek to commission general practice providers to establish four new Medicare Urgent Care Clinics servicing residents in and around the following Local Government Areas (LGAs) and locations: Bayside Stonnington - with a focus on the eastern area and bordering suburbs Mornington Peninsula - with a focus on Hastings Cardinia - with a focus on Pakenham  Please note: locations are subject to change. The upcoming Request for Tender (RfT) will outline the final eligible locations. This tender opportunity is part of the Commonwealth Department of Health, Disability and Ageing's commitment to expand UCCs across the country, opening 50 new clinics nationwide, including 12 in Victoria. SEMPHN expects to open a RfT in mid-late August 2025. Background information Medicare Urgent Care Clinics (UCCs) reduce demand on hospital emergency departments by providing a primary care service for urgent conditions that do not require an emergency response. UCCs are GP-led services that increase access to primary care, providing treatment to people with conditions such as mild infections, sprains, broken bones and minor burns. They also offer pathology and imaging services. UCCs are expected to be open for 14 hours a day, 7 days a week (including public holidays), and are free for people of all ages (with or without a Medicare card). Visit health.gov.au and refer to the Medicare UCC Operational Guidance for more information. Eligibility requirements Detailed eligibility criteria will be available in the RfT documentation. Tender documentation and process SEMPHN will use eProcure as its portal for this RfT opportunity. To apply, interested providers must be registered for eProcure.
