Have your say in SEMPHN's After Hours Needs Assessment

We invite primary care providers to take part in a short survey about the provision of After Hours primary care in south east Melbourne. 

Whether you provide After Hours primary care within the South Eastern Melbourne PHN (SEMPHN) region or not, your insights are extremely valuable in assessing After Hours population and workforce needs, which will inform our service design and commissioning.

Please click on this link for more information and to complete the survey by COB Monday 14 August 2023. It will take approximately 15 minutes. 

  • We request that the survey is completed by someone with knowledge of issues and opportunities of providing services After Hours, particularly if you have previously provided such services but currently do not.
  • The survey will not collect any information that will impact on your relationship with SEMPHN.
