Helping homebound individuals access COVID-19 vaccinations

SEMPHN’s Vulnerable Peoples Homebound Vaccination Program, which helps individuals who cannot leave home access COVID-19 vaccinations, has been extended to 30 June 2026.

To be eligible for the program, the individual must:  

  • be homebound or have a genuine need for an at-home vaccination  
  • be eligible for a COVID-19 vaccination (see eligibility criteria).

Anyone can request a vaccination for an eligible individual using the Vulnerable Peoples Homebound Vaccinations Referral Form, including health professionals, carers and family members. Individuals can also complete the form to request vaccination for themselves.

Once the referral has been received, a vaccination provider will contact the homebound person (or nominated contact person) to schedule a time for the in-reach home visit.
