Helping homebound individuals access COVID-19 vaccinations

SEMPHN’s Vulnerable Peoples Homebound Vaccination Program, which helps individuals who cannot leave home access COVID-19 vaccinations, has been extended to 30 June 2026.

To be eligible for the program, the individual must:  

  • be homebound or have a genuine need for an at-home vaccination  
  • be eligible for a COVID-19 vaccination (see eligibility criteria).

Anyone can request a vaccination for an eligible individual using the COVID-19 Homebound Vaccination referral form, including health professionals, carers and family members. Individuals can also complete the form to request vaccination for themselves.

Once the referral has been received, a vaccination provider will contact the homebound person (or nominated contact person) to schedule a time for the in-reach home visit.
