It is not a substitute for clinical judgment which needs to be responsive to the needs of the individual patients and their circumstances. Responsibility for patient care and decision making, including when recommending a service, rests solely with the treating health professional.

How does SEMPHN obtain referral information?

The referral information is sourced directly from publicly available materials on local hospital websites, including Alfred Health, Peninsula Health and Monash Health. While we endeavour to keep this information current, updates may not always be immediate and differences may arise. For the most accurate and up‑to‑date information, we recommend checking the hospital website.

Monash Health Location Information

SEMPHN notes that Monash Health does not provide physical addresses as part of its online referral information. Location details will be made available during the referral process through Monash Health’s systems by way of referral acceptance letters or emails.
