Expression of Interest: ​​​​​​​Quality improvement activity to increase bowel cancer screening in general practice

SEMPHN is pleased to invite expressions of interest from general practices in the Local Government Areas (LGAs) of Greater Dandenong, Casey and Frankston to participate in a quality improvement activity to improve bowel cancer screening rates.

Why is this program needed?

SEMPHN’s Needs Assessment (2022-2025) indicates that bowel cancer screening rates in these LGAs are among the lowest in the state. 

What is required?

Successful general practice applicants will receive funding to integrate bowel cancer screening processes into their broader practice to increase timely screening and help identify eligible patients. They will be required to:

  • Register their practice with the National Cancer Screening Register (NCSR)
  • Create workflows designed to support the objective and track data
  • Run POLAR reports to identify newly eligible and lapsed patients
  • Work with patients to encourage participation
  • Obtain results and align information with the NCSR
  • Source faecal occult blood test kits as required to provide to patients
  • Provide appropriate patient follow-up as required
  • Commit to providing SEMPHN with relevant project data
  • Commit to continuing this activity post-funding.

How to express your interest

All expressions of interest (EOI) must be submitted through the Folio online application form.

Applicants are required to provide POLAR-generated evidence of the number of patients aged 50 – 74 years with their EOI submission. This data must be de-identified, with further instructions and general practice eligibility provided via Folio.
