How care coordination benefited Jake's diabetes management

New Care Coordination Support Service funded by the SEMPHN helped Jake find a local GP and manage his diabetes in a new state.

Moving interstate to start a new job is a big thing to do. For Jake, it meant leaving behind friends and family and starting again in Melbourne’s south east. At first, he focused on work and joining a local soccer club.

With the move and the new job, Jake hadn’t found himself a new doctor yet.

Jake lives with diabetes and bipolar disorder. He had a few nights where he didn’t quite feel right and ended up at the local hospital. Jake was referred to Silverchain’s new free care coordination support service which helps people living in the Frankston and Greater Dandenong areas to take a proactive approach to managing their health including finding a local GP.

“Silverchain taught me that there is a lot I can do to take better care of myself. They found services where I could get appointments after work. I now have a mental health plan in place and have contacted my local diabetes service. I now know where to get help if I need it,” Jake said.

The new Care Coordination Support Service funded by the South Eastern Melbourne PHN helps to support eligible people who need a little extra help with taking care of themselves. By working closely with local GPs and pharmacies, we are helping to improve health outcomes for vulnerable people, reduce pressure on local hospitals, and empower people to take control of their health.
