Improved access to Multidisciplinary Team Care for patients in Frankston and Mornington Peninsula

South Eastern Melbourne Primary Health Network (SEMPHN) is pleased to announce the successful applicants of the General Practice Multidisciplinary Teams (MDT) initiative.

The GP MDT program aims to improve care for patients with multiple chronic health conditions in the Local Government Areas (LGAs) of Frankston and Mornington Peninsula.  The program will provide funding for practices to expand their teams to include a variety of multidisciplinary professionals, such as allied health professionals, nurse practitioners, nurses, midwives, and Aboriginal health workers or practitioners.

SEMPHN’s 2023 Health Needs Assessment identified Frankston and Mornington Peninsula as priority areas due to a higher proportion of residents with multiple chronic conditions who are disadvantaged and in need of income support.

The program is funded until 30 June 2027.
