Our 2023 Annual Report is now live!

We’re pleased to announce our latest Annual Report is now available to read online.

SEMPHN’s vision is to create opportunities for people in south east Melbourne to live their healthiest lives. Our latest Annual Report (2022-23) showcases how health services and community programs have created positive health outcomes for individuals and communities in our catchment. 

This year we have featured stories and initiatives centring on lived-experience, collaboration and innovation. We also provide insights into the development of our new ‘Path to Impact’ Strategy which will shape the design and delivery of future services and initiatives. 

Our significant impact over the past 12 months and SEMPHN’s work across the sector would not be possible without the involvement of general practices, local hospital networks, allied health services and community members with lived experience contributing to service design and delivery. 

Thank you to all our partners, providers and colleagues for working with us. We look forward to continuing to collaborate throughout this year and into the future.
