Your opportunity to provide feedback – SEMPHN Stakeholder Survey 2023

As a valued stakeholder, SEMPHN would like to ask you a few questions on what we’re doing well and where we can do better, to ensure we can achieve our vision of creating opportunities for people in south east Melbourne to live their healthiest lives.  

We do value your time, and this survey will take less than 10 minutes to complete. 

For every survey response SEMPHN receives, SEMPHN will donate $2 on your behalf to a service (not currently funded by SEMPHN) dedicated to assisting First Nations Peoples or people who are homeless/at risk of homelessness.
