[Closed] Tender: Up to $1.5m to provide Continuum of Care AOD service

SEMPHN is pleased to invite expressions of interest from organisations to pilot a Continuum of Care alcohol and other drug (AOD) service model which includes high and low intensity services in one or more of the following Local Government Areas (LGA): Casey, Port Phillip, and/or Greater Dandenong.

Why is this service needed?

SEMPHN’s Health Needs Assessments between 2019 and 2022 indicate a need for enhanced access and integration of AOD services across south east Melbourne, including the need for more aftercare and post-rehabilitation support services. In addition, for individuals who are experiencing vulnerability due to the multiple complexities associated with their substance use, moving from acute services to outpatient treatment can be challenging and may exacerbate mental health conditions and/or lead to relapse. 

With this in mind, SEMPHN is commissioning the Continuum of Care service model pilot which allows individuals to gradually step down the time and intensity of their treatment while being empowered to return to living independently. 

About the service model

The Continuum of Care AOD Service will offer high intensity and low intensity treatment options via a central ‘hub’ which comprises several wrap-around services. The service will be provided by a multidisciplinary team which may consist of expertise in mental health, primary care, housing, family violence, gambling, employment and pharmacotherapy.

The Continuum of Care approach lessens the likelihood of relapse as it allows tailored and expanded treatment options for individual needs, including more or less intensive treatment over time. 

Two streams of high intensity treatment will be provided as part of this model.
1. Day Program for clients presenting with moderate to severe substance use disorders and multiple complexities.
2. An intensive outpatient program for clients with moderate problematic substance use disorders. 

Low intensity outpatient services will assist clients to maintain their recovery gains by providing continued support through aftercare. Services are designed to allow clients to continue attending work, school and family life while being supported in their recovery journey.
