South Eastern Melbourne Primary Health Network (SEMPHN) is inviting small or solo general practices in Frankston and the Mornington Peninsula to apply for funding to implement Multidisciplinary Team Care (MDT).

This tender opportunity is a chance for eligible general practices to enhance their capacity to provide comprehensive care.

SEMPHN’s 2023 Health Needs Assessment identified Frankston and Mornington Peninsula as priority Local Government Areas due to higher proportions of residents:

  • With multiple chronic conditions and
  • Who are disadvantaged and require income support.

What's involved?

Funding of up to $134,878 (ex GST) over two years based on tiered funding packages will be available for up to 15 general practices to recruit allied health professionals, nurse practitioners, nurses, midwives, or Aboriginal health workers or practitioners. 

The program aims to:

  • Develop and implement multidisciplinary teams that meet the specific needs of the community.
  • Set up reporting processes to monitor progress.
  • Evaluate how these MDT models enhance access to a wider range of primary care services in general practice.

The program's service design involved a co-design process, which engaged eligible small and solo practices in these LGAs to gather input on the preferred local model of care. The feedback indicated a preference for an 'in-reach' approach, meaning referrals and patient care coordination happen within the practice.

Applying for this tender

To be considered for this tender, applicants must:

  • Be a provider which currently delivers general practice services in the LGAs of Mornington Peninsula or Frankston
  • Is a small or solo general practice with 4 or fewer general practitioner FTE roles
  • Be an RACGP Accredited general practice
  • Be able to commence MDT service preparation activities from 1 July 2025, and recruit MDT health professional(s) by 30 September 2025
  • Have a consulting room space/slot available during the day or after hours for the MDT health professional/s.

SEMPHN will prioritise submissions that strongly reflect sustainable approaches to achieving the key program objectives.

  • 1 July 2025: Establish MDT service
  • By 30 September 2025: Recruit MDT health professional(s)
  • 30 June 2027: Contract end date.
