Our 2023-2024 Annual Report is live!

Our latest Annual Report details our accomplishments and highlights the impact of our work on the local community. 

We’ve featured local, SEMPHN-commissioned health services/programs and reflections from individuals who have visited the services or participated in the programs.  

The report may provide useful insights into local services to which you can refer your patients. 

Over the last 12 months, we have achieved many significant outcomes and created an abundance of opportunities for the people of south east Melbourne to live their healthiest lives. 

Some of our key achievements include: 

  • Roll-out of programs responding to aged care, digital health, youth, mental health and AoD needs in our community.    
  • Initiatives that filled service gaps, improved patient experience and created inclusive and accessible environments for health care. 
  • Collaboration with health care providers to deliver targeted staff training and educational resources, leading to enhanced capacity and capability to provide quality care in our region. 

Our impact, and SEMPHN’s work across the sector, would not be possible without the involvement of general practices, local hospital networks, allied health services and the community members with lived experience contributing to service design and delivery.
