2026 Stakeholder Feedback Survey now open

SEMPHN's 2026 Stakeholder Feedback Survey is now open. 

We are inviting stakeholders from across the health and community sector to share their experiences, insights and ideas to help us better understand what is working well, where improvements can be made, and how we can strengthen support, engagement and collaboration across south east Melbourne. 

The survey is an important opportunity for stakeholders to provide feedback on their experiences working with SEMPHN and help inform how we continue to improve our services, partnerships and engagement activities. 

The survey takes less than 10 minutes to complete. 

Why participate? 

Your feedback helps us: 

  • understand stakeholder experiences and perspectives 
  • identify what is working well 
  • identify opportunities for improvement 
  • strengthen partnerships and collaboration 
  • inform future planning and decision-making. 

We are committed to listening and using feedback to continuously improve how we work with providers, partners, communities and the broader health system. 

A survey with impact 

As a thank you for participating, SEMPHN will donate $2 for every completed survey to Foodbank Victoria, supporting programs that help provide fresh, nutritious food to individuals and families experiencing food insecurity across Victoria, including Melbourne's south east. 

Your feedback can make a positive impact both within our primary health system and in our community. 

Your participation not only helps shape SEMPHN's future work, but also contributes to supporting vulnerable members of our community. 

The survey is open until 3 July 2026.
