Opportunity: Advance Care Planning (ACP) Service Visits for General Practice

South Eastern Melbourne Primary Health Network (SEMPHN) is partnering with Alfred Health  and Peninsula Health Advanced Care Planning (ACP) Services to offer general practices the opportunity to connect with their local hospital's ACP Service. 

What is Advanced Care Planning (ACP)? 

ACP is the process of planning for someone's future healthcare and the care they would (or would not) like to receive in the event of serious illness when they are unable to communicate their preferences, or at the end of their life. You can learn more about ACP by visiting our ACP page.

About the opportunity 

You have the opportunity to participate in a 3-4 month program with your local hospital's ACP Service. This program aims to provide information, education, and training to improve the understanding and access to ACP Services. 

To kick off this exciting initiative, SEMPHN is offering an incentive payment of $1,000 (ex GST) per practice to the first ten practices that participate in the program. There are 6 spots remaining in the program. 

What will the program provide?

The program provides practical, evidence-based resources and training that empowers primary care professionals to initiate advanced care planning discussions. An ACP liaison officer will arrange visits to your practice by the local hospital's ACP service at a time that is convenient for you.

The Local Government Areas (LGAs) eligible for this opportunity are:

  • Mornington Peninsula
  • Frankston
