Care finder services: Helping vulnerable Australians find aged care support/services

SEMPHN is pleased to announce the new care finder services for South Eastern Melbourne. Care finder services provide specialist and intensive assistance to help vulnerable, older people to find and connect with aged care health services in their local area. 

 The need for this service was highlighted in the Royal Commission into Aged Care Quality and Safety (2021). 

Across south eastern Melbourne, care finder providers will:

  • Find older people who are disconnected from the health system
  • Support older people to navigate the system and interact with My Aged Care
  • Provide guidance and explain the care assessment process
  • Connect older people with the aged care services or other relevant community supports they need
  • Provide periodic check-ins with clients to ensure they continue to have their needs met. 

Who will be providing the care finder service?

Congratulations to the following organisations which will deliver care finder services:

  • Council on the Ageing Victoria
  • Care Connect
  • Better Place Australia
  • Peninsula Health
  • Housing Aged Action Group
  • Wintringham

We also welcome the following Assistance with Care and Housing providers who will be transitioning to the care finder program:

  • Star Health
  • Sacred Heart Mission
  • MiCare
  • Bayside City Council

The care finder service commenced 3 January 2023 and is available across South Eastern Melbourne.
