General Practices invited to complete Digital Health Maturity Assessment

SEMPHN is inviting general practices across the catchment to complete the Digital Health Maturity Assessment tool. 

SEMPHN currently provides a range of digital health support to local practices and we would like to understand how digital health is used in practices everyday operations. The assessment also provides the opportunity to inform us on how we can better support general practice and their specific individual needs. 

The assessment will open on 1 October 2024 and will be available for a limited time. Every general practice in the SEMPHN catchment will have the opportunity to complete the assessment at no cost.    

What is a Digital Health Maturity Assessment? 

The assessment tool provides an insight into how and when general practices use digital health activities, providing guidance for future activities. 

The DHMA Tool has been developed by Semantic Consulting and the Digital Health Team at SEMPHN. 

Why complete the assessment tool?

The purpose of this tool is to better understand where general practices are at on their digital health journey, providing guidance to both SEMPHN and the general practices to support future planning to enable digital enhancements. 

What are the benefits to General practice? 

  • Understanding of current digital maturity 
  • Understand current business performance in digital health and identify next steps 
  • Identify opportunities for digital enhancements 
  • Provide evidential support for grants and tender applications  
  • Risk management tool to identify digital vulnerabilities about cyber security and disaster recovery   
  • Receive Tailored Action Plan based on your responses with links to relevant educational resources to complete at your own pace  
  • Provide evidence of continuous quality improvement for accreditation purposes.  

What are the benefits to SEMPHN? 

In completing the assessment tool, practices will help SEMPHN develop a snapshot of the 2024 digital health landscape and allow us to identify levels of digital health maturity. In turn, SEMPHN can develop plans and activities targeted at the needs and capabilities of individual clinics and/or communities rather than a one-size-fits-all approach to developing digital health capacity. 

How will the data be used? 

All information collected will be visible only to the general practice completing the assessment, SEMPHN, and the contracted provider  (Semantic Consulting). SEMPHN will store and use responses provided for the purpose of the evaluation of digital health use within south east Melbourne, and to support the delivery of targeted support initiatives. Data aggregated across the whole region may be used in publications (e.g. the SEMPHN annual report and health needs assessment) however individual practices will never be identified outside of previously described purposes.

To assist practices in completing the assessment, please join one of our drop-in guided walk-through sessions to learn how to complete the plan.

  • Session 2:  Tuesday 15 October 2024 (3.30pm - 4:30pm) 
  • Session 3: Wednesday 23 October 2024 (8.30am - 9:30am)

South Eastern Melbourne Primary Health Network (SEMPHN) will soon seek to commission general practice providers to establish four new Medicare Urgent Care Clinics servicing residents in and around the following Local Government Areas (LGAs) and locations: Bayside Stonnington - with a focus on the eastern area and bordering suburbs Mornington Peninsula - with a focus on Hastings Cardinia - with a focus on Pakenham  Please note: locations are subject to change. The upcoming Request for Tender (RfT) will outline the final eligible locations. This tender opportunity is part of the Commonwealth Department of Health, Disability and Ageing's commitment to expand UCCs across the country, opening 50 new clinics nationwide, including 12 in Victoria. SEMPHN expects to open a RfT in mid-late August 2025. Background information Medicare Urgent Care Clinics (UCCs) reduce demand on hospital emergency departments by providing a primary care service for urgent conditions that do not require an emergency response. UCCs are GP-led services that increase access to primary care, providing treatment to people with conditions such as mild infections, sprains, broken bones and minor burns. They also offer pathology and imaging services. UCCs are expected to be open for 14 hours a day, 7 days a week (including public holidays), and are free for people of all ages (with or without a Medicare card). Visit health.gov.au and refer to the Medicare UCC Operational Guidance for more information. Eligibility requirements Detailed eligibility criteria will be available in the RfT documentation. Tender documentation and process SEMPHN will use eProcure as its portal for this RfT opportunity. To apply, interested providers must be registered for eProcure.
