Learn about SEMPHN’s impact in our 2024–25 annual report

We’re pleased to announce SEMPHN’s 2024–25 annual report, Our Impact: Report to the Community, is now available to read online. 

SEMPHN’s vision is to create opportunities for people in south east Melbourne to live their healthiest lives. Our report showcases how health services and community programs have created positive health outcomes for individuals and communities in our catchment.

This report covers the second year of our 5-year Path to Impact strategy and marks the end of SEMPHN’s first decade of operation. It features stories and initiatives centering on the 7 priority areas set by the Australian Government for primary health networks:

  • mental health
  • First Nations health
  • population health
  • health workforce
  • digital health
  • aged care
  • alcohol and other drugs.

Our impact over the past 12 months and SEMPHN’s work across the sector would not be possible without the involvement of general practices, urgent care clinics, allied health providers, pharmacists, local hospital networks, and community members with lived experience contributing to service design and delivery.

Thank you to all our partners, providers, and colleagues for working with us. We look forward to continuing to collaborate throughout this year and into the future.

Funded by South Eastern Melbourne Primary Health Network (SEMPHN), Silverchain’s Care Coordination Support Service (CCSS) is supporting general practices across south east Melbourne to better care for patients with chronic and complex needs.
