SEMPHN’s care coordination funding can help and the application process has never been easier.

What is care coordination?

Care coordination involves working with a patient to develop a comprehensive plan that helps the patient take more control of their health and achieve their goals.

Care coordination is for patients with a chronic condition(s), at risk of admission to hospital, or who may have complex needs. It is a patient-centred approach that involves the timely coordination of health, community and social services to meet a patient’s needs.

How can care coordination make a difference to my practice?

Care coordination funding can support extra staffing or extra time for your practice. This means more time can be dedicated to supporting your patients and to further refining processes, recalls and reminders.

Did you know most practices that received care coordination funding from SEMPHN have continued care coordination in their practice beyond the funding period? In short, care coordination works!

Who is eligible for the funding?

  • General practices in the local government areas of Bayside, Cardinia, Casey, Frankston, Glen Eira, Greater Dandenong, Kingston, Mornington Peninsula, Port Phillip, and Stonington can apply. 
  • General practices with up to 5 full-time equivalent (FTE) general practitioners (up to 50 GP sessions available per week). General practices with more than 5 FTE GPs are not eligible.
  • General practices which have previously not been successful in their applications are encouraged to apply again. Our simplified application process makes it easier to apply than in previous rounds. 

General practices which have previously received care coordination grants (CC1, CC2 or CC7) from SEMPHN are not eligible.

  • All questions about the tender must be submitted through SEMPHN’s eProcure.
