Does your general practice have 5 or less full-time equivalent GPs?
Is your general practice interested in providing care coordination, but you haven’t had the time or resources?
SEMPHN’s care coordination funding can help and the application process has never been easier.
Care coordination involves working with a patient to develop a comprehensive plan that helps the patient take more control of their health and achieve their goals.
Care coordination is for patients with a chronic condition(s), at risk of admission to hospital, or who may have complex needs. It is a patient-centred approach that involves the timely coordination of health, community and social services to meet a patient’s needs.
How can care coordination make a difference to my practice?
Care coordination funding can support extra staffing or extra time for your practice. This means more time can be dedicated to supporting your patients and to further refining processes, recalls and reminders.
Did you know most practices that received care coordination funding from SEMPHN have continued care coordination in their practice beyond the funding period? In short, care coordination works!
Watch this short video to learn how a practice (and their patients) have benefited from SEMPHN care coordination funding.
General practices which have previously received care coordination grants (CC1, CC2 or CC7) from SEMPHN are not eligible.
