Accredited general practices invited to join Hepatitis C Quality Improvement activity

SEMPHN is offering accredited general practices to take part in a supported 4-month Hepatitis C Quality Improvement project.

 

Did you know Hepatitis C affects 65,000 Victorians and if untreated leads to multiple health issues such as chronic liver disease?

In 2016, only 1% of people with Hepatitis C were being treated. Fortunately, recent changes in Hepatitis medication management has led to a 95% success rate of the virus being cured after an 8–12 course of medication. It is envisioned that by 2030, 90% the proportion of people living with chronic hepatitis will be cured of the virus.

This Quality Improvement activity, focusing on Hepatitis C, supports general practices to identify and screen who may be at risk for Hepatitis C.

Why get involved?

  • Practices receive $2000 to support the Practice Nurse lead’s participation
  • This activity supports practices to identify patients who are at risk of developing Hepatitis C
  • Learn more in education workshops run by VHHITAL at the start, middle and end of the project
  • This activity offers support and guidance for practices to develop monthly Plan Do Study Act (PDSA) cycles
  • The activity fulfils your PIP QI accreditation requirements
  • Clinicians involved receive CPD points for education activities 

Who should apply?

We are looking for practices with patients who:

  • Are at risk of contracting Hepatitis C, including lifestyle behaviours such as sharing needles and drug use equipment
  • Have had unsterile tattooing or body piercing
  • Engage in high-risk sexual behaviours including sex work
  • Have been infected with HIV or Hep B
  • Clinical evidence of liver disease such as persistently elevated ALT
  • Have spent time in custodial setting (prison)
  • Have immigrated or are refugees from Egypt, Pakistan, Mediterranean, Eastern Europe, Africa or Asia

Eligibility

  • General practices must be accredited to participate in the activity
  • General practices must commit to the project time frame (4 months)
  • A designated practice nurse must be the point of contact to meet all the project requirements.
