[Closed] Up to $2 million available to provide a Care Coordination Support Service

SEMPHN is pleased to invite services to tender to deliver an innovative Care Coordination Support Service (CCSS) which will link eligible patients from Frankston and Greater Dandenong to appropriate clinical and non-clinical services.

Why is this service needed?

SEMPHN’s 2021 Needs Assessment identified that Frankston and Greater Dandenong had the highest number of lower-urgency, after hours emergency department hospital presentations in the South Eastern Melbourne catchment.

About the service

The Care Coordination Support Service provider will be required to work with general practices, community pharmacies and public hospitals. 

The Care Coordination Support Service aims to support the wellbeing of eligible people, reduce presentations at emergency departments and reduce preventable hospital admissions, particularly during the after hours period, by:

  • Working with general practices and community pharmacies
  • Supporting general practices to provide patient-centred care through comprehensive care coordination activities 
  • Linking people who don’t have a regular GP, and are more likely to access emergency department services, to general practice services
  • Developing and implementing a pharmacy-based after hours awareness initiative to increase consumer awareness of local after hours services
  • Developing a potential referral pathway involving public hospitals.
